Abstract: 

Cancer pain is one of the most common reasons for ED visits and hospitalization and has been linked to increased 30-day mortality. Opioids are necessary in almost all patients for the management of their pain. Despite that, cancer pain is inadequately treated in approximately 56 to 82% of patients. Using a multidisciplinary approach, minimally invasive therapies can help relieve pain and improve quality of life.

 

Session Objectives:

  1. Describe pathophysiology of cancer pain
  2. Discuss treatment guidelines for adult cancer pain
  3. Understand the role of IR interventions in palliative care
  4. Review outcome data of IR interventions for cancer pain

Abstract:

Breast radiologists represent the face of imaging 3.0 and are key players in advocating for patients to reduce these disparities. Diversity-related barriers for breast imaging patients, as they journey from screening to survivorship, include impediments to access and quality of care, gaps in communication, and lack of knowledge in both providers and patients. Potential strategies for overcoming these specific barriers include “culturally tailored” nurse navigators, mobile mammography, improved communication, patient and provider education, and breast radiologist involvement in advocacy efforts promoting diversity.

 

Objectives:

  • To describe the diversity in outcomes of breast cancer in different US populations
  • To describe non-genetic factors that contribute to diversity in outcomes in breast care
  • To describe diversity in patient care along the continuum of breast imaging
  • To describe strategies to address disparities in breast imaging care

Jeffrey Weinreb, MD is Professor of Radiology and Biomedical Imaging at the.  For four decades, he pioneered a wide range of MRI techniques and clinical applications, and he has been a leader in the international effort to develop PI-RADS (Prostate Imaging Reporting and Data System) to standardize the acquisition, interpretation, and reporting of prostate MRI. 

 

Dr. Weinreb has had numerous leadership positions in professional societies, including Chair of the American College of Radiology (ACR) Commission on Quality and Safety, Vice President of the ACR, President of the New York Roentgen Society, and President of the Society of Advanced Body Imaging (SABI). He has received numerous honors, including Fellow of the International Society of Magnetic Resonance in Medicine (ISMRM) and Gold Medal Awards for lifetime achievement from the ACR and SABI.

Abstract

Breast, cervical and ovarian cancer imaging, affect patient management.  This lecture will delve into the challenges of imaging each of these organs and what we’ve recently accomplished to overcome imaging hurdles.  The focus will be on technical advances achieved with MRI, including advanced diffusion MRI with Restriction Spectrum Imaging.  Our group is working on a high resolution non-contrast breast MRI screening tool, a more accurate staging technique for cervical cancer, and trying to break barriers and develop a useful screening tool for ovarian cancer in high risk women.

  1. Define women’s imaging
  2. Review Restriction Spectrum Imaging (RSI)
  3. Discuss progress and organ specific adaptions of RSI to breast, cervix, and ovarian imaging

In the last two decades, our military and federal health care facilities have transitioned from traditional X-rays exposing film screen systems, developed much like photographic film, to an entirely digital detection system that allows for computer processing of images and digital image and report distribution.

 

In this webinar, we will outline how application of physical principles of X-rays, with digital detectors and computer data manipulation, can present images demonstrating chest and heart diseases that were previously not readily visible by traditional film screen systems.

Abstract: Informed consent is ubiquitous in clinical workflows, particularly more procedural specialties, and is at the core of our relationship with patients. Despite its importance, studies have repeatedly shown consent practices to be limited in healthcare, and radiology is not immune to these challenges. Drs. Isabel Newton and Eric Keller have been working through a not-for-profit organization to improve consent practices for image-guided procedure and have developed a practical ICONSENT curriculum to better engage the radiology community about these issues. In this grand rounds, Drs. Newton and Keller will discuss the practical challenges of ideal consent practices in radiology workflows as well as introducing a framework to improve consent practices in radiology and beyond.

LECTURE:            Stability, STIR and SINS: Imaging Metastatic Disease in the Spine

 

ABSTRACT

Up to 70% of patients with systemic malignancy will develop spinal metastatic disease during the course of their illness, with a wide-range of potential complications including pain, prolonged disability and cord compression.  As such, early diagnosis of spinal metastatic disease is key and provides tremendous opportunity for the practicing radiologist to add value to clinical treatment decisions.  In this setting, the aims of the present talk include:

 

  1. To familiarize the audience with multi-modal imaging of metastatic disease, including a discussion of the strengths and weaknesses of radiographs, CT and MR
  2. To discuss the role of imaging in radiotherapeutic and surgical treatment strategies
  3. To introduce the Spinal Instability Neoplastic Score (SINS)

The design process for any new disruptive technology, including Artificial Intelligence solutions, critically shapes the ultimate clinical outcome of the tool. Embedding diverse thought leadership and considering many differing perspectives ultimately shapes the way a tool impacts patient care. Technology, such as artificial intelligence, has the potential to either deeper or lessen healthcare inequities. By carefully examining the technology design process, we can begin to understand how this technology can change the way we practice medicine and deliver excellent care to our heterogeneous patient populations. 

 

Session objectives: 

1.            Explore the concept of diversity and its relationship to healthcare inequity.

2.            Talk about how diversity is or is not woven into the AI design process.

LECTURE:            Breast cancer screening controversy: update with recent CNBSS revelations

 

ABSTRACT

Eight randomized controlled trials (RCTs) were conducted in the 1960s to 1980s to evaluate the efficacy of screening mammography to decrease mortality from breast cancer [1-7]. All trials confirmed significant benefits of screening mammography except the two in the Canadian National Breast Screening Study (CNBSS), CNBSS-1 for women 40-49 and CNBSS-2 for women 50-59. In fact, a slight mortality increase was noted in the CNBSS-1 trial in the screening arm at 11-14 years of follow-up [8]. Due to the large size of the CNBSS-1 trial and the specific targeted age range of participants, it continues to heavily influence guidelines for breast cancer screening[15] of women age 40-49[10, 16]. Multiple experts have hypothesized that the results of CNBSS-1, indicating no benefit or even possible harm related to screening mammography, reflect both design flaws and the failure to adhere to the standards of RCTs during day-to-day operations of the CNBSS[17-21].  The purpose of this presentation is to consider if the CNBSS trials were valid studies of breast cancer screening in the era in which they were conducted. And, if so, do their findings translate to current screening practice?

 

OBJECTIVES

1. review the basic standards of RCTs 

2. critically consider new, eyewitness accounts of the daily management, operations, methodology regarding inclusion and exclusion criteria, and randomization during the CNBSS 

3. determine if the CNBSS remains scientifically valid for establishing population screening guidelines

LECTURE:            MRI of the Spine in Axial Spondyloarthritis

In this lecture, the imaging appearances of spondyloarthritis in the spine, especially on MRI will be reviewed. The appearance and distribution of active inflammatory disease and structural damage changes will be discussed in comparison to differential diagnoses, especially disc degeneration and DISH, as well as other conditions. The lecture will conclude with a brief discussion of Classification of Axial Spondyloarthritis.

 

Objectives:

At the end of the lecture, participants will be familiar with common and uncommon spinal imaging findings of axial spondyloarthritis, be able to distinguish axSpA from other conditions, and be aware of some pitfalls of interpretation and the limitations of imaging and classification.

The idea of Total Body PET was first suggested in 1993. However, it was not until the turn of the century that effective technologies were available to bring this idea to reality, and it was not until 2015 that funds became available to build the first prototype. This presentation will briefly describe the history of the project, and will present some of the benefits, challenges and opportunities presented by Total Body PET.

BIO

Dr. Badawi is Professor and Vice-Chair for Research in the Department of Radiology, and Professor of Biomedical Engineering at UC Davis. With Professor Simon Cherry, he co-Directs the EXPLORER Molecular Imaging Center and co-leads the EXPLORER Consortium, which designed and built the world’s first total-body PET scanner. Dr. Badawi obtained his PhD in PET Physics at Guy’s and St. Thomas’ Hospital in London. After a postdoctoral fellowship at the University of Washington, he joined the Dana Farber Cancer Institute in Boston in 2000, where he helped to set up their first clinical PET service. In 2004 he joined UC Davis. Dr. Badawi’s lab developed the world’s first dedicated breast PET/CT scanner, and a low-cost ultra-high resolution scanner for mouse extremities that matched the highest resolution then achieved for in vivo PET imaging. Bio:

Paul Roben, Ph.D., was named Associate Vice Chancellor for Innovation and Commercialization at UC San Diego in June 2015.

 

Working with the Vice Chancellor for Research, Dr. Roben is transforming the functions provided by the Technology Transfer Office, Industry Research Alliances, and Industry Contracting into an integrated organization to include patenting, strategic corporate research partnering and industry contracts. He oversees the Office of Innovation and Commercialization, which will establish a campus-wide innovation platform to build a systemic and sustainable innovation culture, create a vibrant regional ecosystem, and accelerate the commercialization of UC San Diego inventions.

Previously he was Senior Director of Office Technology Development at the Salk Institute in La Jolla. 

 

As a key member of Ireland’s Innovation Taskforce (2009 to present), he co-authored a national economic-development plan for Ireland, among other achievements.

Dr. Khurana completed his radiology residency at UCSD from 2014-2018 after his postdoctoral fellowship at Stanford University in Molecular Imaging. He graduated from Body Imaging fellowship at Stanford University in 2019 and then started a faculty position at the University of Kentucky.  Dr. Khurana was the recipient of Elliott C. Lasser Award for Excellence in Research presented at his Radiology Residency Graduation in 2018 and has 26 published peer-reviewed articles in fields of Radiology and Molecular Imaging. His current research interests include Placenta Accreta imaging, T-cell labeling and tracking, micro-learning & US LI-RADS.

Abstract:

Organization, characterized by fibroblast proliferation, is a common and nearly universal response to lung injury whether it is focal or diffuse. Despite the vast range of injurious agents, the lung’s response to injury is quite limited, with a similar pattern of reaction seen radiologically and histologically regardless of the underlying cause. Although there is a tendency to divide organization into distinct entities, the underlying injury to the alveolar epithelial basement membrane is a uniting factor in these processes. This pattern of lung injury is seen in the organizing phase of diffuse alveolar damage, organizing pneumonia (OP), acute fibrinous and organizing pneumonia, and certain types of fibrotic lung disease. In addition, although organization can heal without significant injury, in some instances it progresses to fibrosis, which can be severe. When fibrosis due to organization is present, other histologic and imaging patterns, such as those seen in nonspecific interstitial pneumonia, can develop, reflecting that fibrosis can be a sequela of organization. This talk reviews the histologic and radiologic findings of organization in lung injury due to diffuse alveolar damage, OP, and acute fibrinous and organizing pneumonia and helps radiologists understand that the histologic and radiologic findings depend on the degree of injury and the subsequent healing response.

 

Objectives: 

  1. Understand that the main pathologic patterns of COVID-related lung injury are diffuse alveolar damage and organizing pneumonia
  2. Learn how the alveoli heal after lung injury
  3. Discover how fibrosis develops if the normal healing response is incomplete or dysregulated

On behalf of the Physics Department at SDSU, I (Usha Sinha, Chair of Physics), would like to invite you all for the annual Doc Morris Public Lecture on April 22, 2022.  This year we are honored to have Dr Peter Basser, Associate Scientific Director for Imaging and Genomic Integrity, NICHD IRP, as the speaker.  His talk is entitled, “Invention of Diffusion Tensor Imaging (DTI) and a Glimpse of What's Ahead”.  Details of the talk and a reception before the talk are included in the enclosed flyer. Peter Basser is a legend in DTI and microstructure imaging and needs no introduction. He is widely known for the invention, development, and clinical implementation of MR diffusion tensor imaging (DTI), diffusion tensor "stream line tractography," and other quantitative MRI methods for performing in vivo MRI histology or "microstructure imaging".  Dr. Basser was the recipient of the Gold Medal from the International Society of Magnetic Resonance in Medicine (ISMRM).

 

I wanted to add a few words about Doc Morris in whose honor we have this annual lecture. Doc got his PhD in Physics from UC Berkeley and almost right after, joined the SDSU Physics Department.  He was a brilliant physicist who devoted 60+ years to teaching.  He taught an Advanced Physics Lab class that was the capstone lab for our majors-- each experiment required thought and work equivalent to a mini-thesis and each experiment was modeled after a Nobel Prize winning experiment.  He challenged the students to strive higher and decades later, generations of students recall his lab as the single most important course in their entire academic life-- that his lab made them choose Physics for their future career. In fact, the current Chair of Physics at UCSD, Prof. Maple, is a former student of Doc Morris.  Doc volunteered and taught four years even after retirement-- he was 87 then (he could be seen setting up the experiments late in the evening).  He was a legend- I am glad I had the privilege of knowing him.

Dr. Daniel Lee has been the Nuclear Medicine Section Chief for Ochsner Medical Center in New Orleans,

LA since March 2019. He is also the Nuclear Medicine Medical Director for Ochsner Clinic Foundation

and the Director of Peptide Receptor Radionuclide Therapy for NOLANETS, a multidisciplinary group

specializing in the care of neuroendocrine tumor patients. He has two decades of experience in nuclear

Abstract: 

Radiology has traditionally lived at the center of technology innovation, from pioneering in its early days of innovation in cardiovascular intervention and surgery, led by many interdisciplinary giants. As new disciplines of angiography, CT, and MRI emerged, these early pioneers recognized the weaknesses in the medical practices of the day, developed and applied new technologies to the betterment of the care of their patients -- which have become today's standards of care. Engineering and advanced computation are two of the new disciplines of the modern era, which undoubtedly will transform medical practice over the coming decade. I will present our lab's work innovating at the intersection of these new fields and clinical cardiac and thoracic imaging, which help to place UC San Diego at the forefront of clinical care.

 

At the end of the presentation, attendees will:

•             Improve familiarity of methods for developing and evaluating imaging artificial intelligence (AI) algorithms.

•             Improve familiarity with applications of imaging artificial intelligence currently in development and use at UCSD.

BIO: 

Albert Hsiao, MD, PhD is a cardiothoracic radiologist trained in engineering at Caltech and bioengineering and bioinformatics in the UC San Diego Medical Scientist Training Program (MSTP). He completed his residency and fellowships in Interventional Radiology and Cardiovascular Imaging at Stanford before returning to UC San Diego as faculty in Radiology, where he leads advanced cardiovascular imaging and the Augmented Imaging and Data Analytics (AiDA) research laboratory. He also serves as co-director of the T32 clinician-scientist radiology research residency program and co-director of the MSTP SURF program. While a radiology resident himself at Stanford, he co-founded Arterys, a cloud-native software company to bring 4D Flow MRI and artificial intelligence technologies to market. He continues to partner with industry to create and bring new imaging technologies to improve diagnosis and management of disease.

Abstract:

One of the most promising areas in medicine today is the diagnosis and treatment of breast cancer. Due to the non-invasive nature of medical imaging, it continues to be an integral part of disease diagnosis and treatment decisions. With the addition of biologic factors in the American Joint Committee Cancer Staging Manual (8th edition), we are in a new era of defining breast cancer. For radiologists to continue to add value to the multidisciplinary approach of patient centered care, they need to understand the molecular subtypes of invasive breast cancer. This presentation will discuss the different subtypes and explain the incidence, imaging appearance, treatment response and overall survival of the patient.

Abstract

Over the last fifty years healthcare has undergone significant digital transformation, with computerized workflows constituting a major part of patient care. Reliance on technologies such as electronic health records (EHR), digital picture archiving and communication systems (PACS), laboratory information systems (LIS), and connected medical devices has grown to supplant manual paper-based systems for many healthcare delivery organizations (HDOs) across the globe. Inevitable flaws in the software and hardware of computer systems can result in vulnerabilities that alter the intended function of a system. Cyber attackers can exploit these vulnerabilities to steal information, disrupt services, or corrupt data. The protection of systems from cyber-attacks is called cybersecurity and is an increasingly important practice for modern healthcare.  

 

Objectives:

1.            Become familiar with hacking concepts such as vulnerability, exploit, CIA Triad, etc.

2.            Understand how cyber security vulnerabilities can lead to breaches of protected health information and patient safety/care quality concerns.  

 

BIO:
Dr. Christian Dameff is an Emergency Physician, Clinical Informaticist, and researcher. Dr. Dameff is also a hacker and security researcher interested in the intersection of healthcare, patient safety, and cybersecurity. He has spoken at some of the world’s most prominent hacker forums including DEFCON, RSA, Blackhat, Derbycon, BSides: Las Vegas, and is one of the cofounders of the CyberMed Summit, a novel multidisciplinary conference with emphasis on medical device and infrastructure cybersecurity. Published cybersecurity topics include hacking 911 systems, HL7 messaging vulnerabilities, and malware.

Theranostics with Radiation-induced Ultrasound Emission (TRUE)

 

Prof. Liangzhong (Shawn) Xiang

ABSTRACT: We explore new ways to generate ultrasound for theragnostics. Specifically, we use varies radiations (X-ray, proton, laser, and electrical field) to treat diseases, simultaneously it will produce ultrasound waves for image-guided interventions. This talk will cover three primary research areas: 1) X-ray-induced acoustic computed tomography (XACT) for precision radiotherapy; 2) Proton-induced acoustic imaging for proton therapy guidance, and 3) electroacoustic tomography (EAT) for irreversible electroporation (IRE) monitoring. Prospects and challenges for the clinical implementation of these techniques will be discussed. The successful development of these technologies will expand the current clinical paradigm towards precision medicine.

 

BIO: Liangzhong (Shawn) Xiang is a tenured associate professor with joint appointments in the Departments of Biomedical Engineering and Radiology, and Faculty Innovation Fellow at University of California, Irvine (UCI). He is also a core faculty member in Beckman Laser Institute and Medical Clinic, and Chao Family Comprehensive Cancer Center at UCI. Dr. Xiang’s lab focuses on biomedical imaging and image-guided treatment. Before joining UCI, he was awarded the Lloyd G. and Joyce Austin Presidential Professorship at The University of Oklahoma (OU). Prior to OU, he completed his postdoctoral fellowship training in medical physics at Stanford Medical School where he was awarded the DoD Prostate Cancer Postdoctoral Fellowship.

 

Dr. Xiang is the recipient of the NIH MERIT Award (R37), and Research Scholar from the American Cancer Society. Dr. Xiang has served as conference chairs for the 2021 IEEE IUS, 2019 AAPM annual meeting and 2018 CIOP. His clinical interests include aortic aneurysms, venous diseases, vascular malformations, and image guide treatment of liver cancers and uterine fibroids. He is also actively involved in clinical research in these areas.

Title:  Imaging cerebral hemodynamics in typical and abnormal aging

Abstract: White matter plays a critical role in supporting human cognitive function, and white matter lesions have been implicated as a contributor to cognitive decline in Alzheimer’s disease (AD). While they are often presumed to be of vascular origin, physiological mechanisms underlying these lesions remain unclear. As the world’s population continues to age, understanding microvascular hemodynamics in typical versus abnormal aging could be critical for understanding vascular contributions to cognitive decline and for guiding the development of preventative therapies. During this talk, I will discuss 1) recent magnetic resonance imaging (MRI) approaches for characterizing gray and white matter hemodynamics in typical aging and 2) potential microvascular mechanisms that may contribute to the development of white matter lesions and consequent cognitive decline in older adults at elevated risk for AD.

 

Bio: Meher Juttukonda, Ph.D. is an Instructor in Radiology at Harvard Medical School and Massachusetts General Hospital and a faculty member in the Athinoula A. Martinos Center for Biomedical Imaging. Meher received a B.E. in Biomedical Engineering from Vanderbilt University and a Ph.D. from the Joint Department of Biomedical Engineering at UNC-Chapel Hill and NC State University. He completed a postdoctoral fellowship at Vanderbilt University Medical Center, studying hemodynamic sequalae of sickle cell disease and arterial steno-occlusive disease. His current research is focused on understanding microvascular mechanisms associated with white matter lesions in typical aging and in adults at elevated risk for AD.

Lecture Title: Augmented Reality: potential and impact of using body donors’ CT scans in medical training

 

 

Abstract and Session Objectives:

Augmented reality (AR) has recently been utilized as an integrative teaching tool in medical curricula given its ability to view virtual objects while interacting with the physical environment. The evidence for AR in medical training however, is limited. For this reason, the purpose of this mixed methods study was to evaluate the implementation of overlaying donor-specific diagnostic imaging (DSDI) onto corresponding body donors in a fourth-year, dissection-based, medical elective course entitled Anatomy for Surgeons (AFS). Students registered in AFS were separated into groups; either receiving DSDI displayed on Microsoft HoloLens AR head-mounted display (n = 12), or DSDI displayed on iPad (n =15). To test for change in spatial ability, students completed an anatomical mental rotation test (AMRT) prior to and following the AFS course. Students also participated in a focus group discussion and completed a survey at the end of AFS, analyzed through thematic triangulation and an unpaired, Mann Whitney U test respectively, both addressing dissection experience, DSDI relevancy to dissection, and use of AR in anatomical education. Although statistically significant differences were not found when comparing student group AMRT scores, survey and discussion data suggest that the HoloLens had improved the students’ understanding of, and their spatial orientation of, anatomical relationships. Trunk dissection quality grades were significantly higher with students using the HoloLens. Although students mentioned difficulties with HoloLens software, with faculty assistance, training and enhanced software development, there is potential for this AR tool to contribute to improved dissection quality and an immersive learning experience.

Lecture Title: Developing a Spine Interventional Service: Beyond LP's & Myelograms

 

Presentation Summary:

 

The aim of this talk is to provide an overview on how to develop and grow a successful spine procedural service, one that goes beyond lumbar punctures and myelograms. Pertinent topics that will be covered include tips on building a procedural team, type of imaging modality to use (i.e. fluoroscopy, CT, or US), how to successfully obtain referrals from clinicians, measuring outcomes, and developing a patient friendly service. Particular emphasis will also be placed on procedural safety and dealing with complications.

The coronavirus disease 2019 (COVID-19) pandemic has had a profound impact on radiology in the United States. The subspecialty of breast imaging is no exception, and the patient-facing nature of the field poses unique challenges to safe care delivery and education during the pandemic. This lecture will discuss the adaptations of breast imaging centers to the pandemic and the impacts on breast imaging education, with a focus on the early portions of the pandemic. The lecture will also explore how lessons from breast imaging may be applied to other radiology subspecialties.

1. Summarize the impact of the COVID-19 pandemic on breast imaging volumes and discuss the resulting financial impact on physicians and staff.

Bio:

Dr. Tutton received his degree from the New Jersey School of Medicine and completed his internship, residency and fellowship at the Feinberg School of Medicine, Northwestern University in Chicago IL.    He joined the Medical College of Wisconsin in 2005 where he established his practice focusing on palliative interventional therapies in cancer and non-cancer musculoskeletal diseases. Dr. Tutton received board certification in Hospice and Palliative Care in 2012. He holds joint faculty appointments in Orthopedics and Palliative Medicine.

 

Dr. Tutton is an active member of the Sarcoma Disease Team at MCW focusing on bone and soft tissue tumor related pain and skeletal complications. His research focus is on ablation of soft tissue tumors including Desmoids and ablation and stabilization for patients suffering from painful cancer related spine and pelvic fractures.

Abstract:

Desmoid Fibromatosis/ Desmoids are benign locally aggressive tumors, affecting younger patients, occurring sporadically or less commonly associated with familial polyposis syndrome (FAP/Gardner’s syndrome). They occur along facial planes and aponeurotic tissue. This orphan tumor is grouped with the sarcomas as it is a soft tissue tumor of fibroblast origin. Historically, surgery was the chosen therapy, but recurrences have been seen in up to 40% in some series. Currently, surveillance is the preferred therapy in asymptomatic tumors that are not progressing rapidly. More recently, percutaneous ablative therapies have become utilized in symptomatic tumors as a “least worst” option over cytotoxic chemotherapy, radiation, and surgery in this young active population. We will review the current imaging, and treatment strategies of desmoids and recent changes in the NCCN guidelines.

 

Objectives:

  • Learner will be able to describe preferred imaging modalities and features of Desmoid Fibromatosis (DF) suggesting active aggressive disease.
  • Learner will be able to describe the recent NCCN guidelines for work-up and treatment of DF.

Lecture Title: Understanding the mechanisms of high altitude pulmonary edema: new insights from proton MRI

 

Abstract & Objectives:

 

High-altitude pulmonary edema, HAPE, is a potentially fatal lung edema that affects individuals who travel rapidly to high altitude. Although many individuals making a rapid ascent will develop findings consistent with interstitial pulmonary edema, 2% of subjects develop alveolar flooding consistent with HAPE, requiring treatment. The biggest predictor of development of HAPE on future ascents  is a prior history of the disease, and these individuals are considered to be HAPE-susceptible.  This presentation will focus on how functional magnetic resonance imaging has provided insights into the mechanism(s) of HAPE. For decades it was proposed that HAPE results from uneven hypoxic pulmonary vasoconstriction of precapillary arterioles increasing resistance and reducing flow in some segments and increasing pressure and flow in non-constricted segments. This in turn leads to mechanical stress failure of pulmonary capillaries and a subsequent inflammatory response exacerbating the leak. Studies using arterial spin labeling to measure perfusion have shown that pulmonary blood flow becomes more heterogeneous when HAPE-susceptible subjects but not HAPE resistant or normal controls are exposed to acute isobaric hypoxia. This provides evidence that uneven hypoxic pulmonary vasoconstriction is indeed a feature of those that are HAPE-susceptible. One explanation for uneven hypoxic pulmonary vasoconstriction is increased heterogeneity of ventilation leading to an uneven stimulus to the response. Additional work using an oxygen enhanced ventilation MR technique, specific ventilation imaging, has shown that contrary to predictions, ventilation is more uniform in HAPE-susceptible subjects compared to HAPE-resistant subjects. This implicates abnormalities in the distribution of vascular smooth muscle as the most likely underlying abnormality in those susceptible to HAPE.

 

The objectives of this presentation is to provide a foundation for how functional lung imaging can be used to understand mechanisms of disease.

Lecture Title: Applied Ethics in Radiology: Practical Solutions to Challenging Situations

 

Abstract: Radiology, like all fields medicine, has challenging ethical situations that can arise in daily practice. Traditional approaches to ethics tend to be general and ’top-down’ based on abstract principles that can feel distant from daily practice in radiology. Conversely, applied ethics is a bottom-up approach where one explores perceptions and experiences surround salient ethical issues and uses this information in tandem with ethical theory to develop practical tools and workflows to navigate these challenging situations better. Dr. Keller and his team has been trialing this approach in radiology, particularly interventional radiology as means of making ethics more useful in daily practice. In this session, we will explore at least four issues relevant to radiology, including futility, consent, research ethics, and artificial intelligence. Each topic will start with a case allowing the audience to anonymously respond what they would do. We will then discuss each issue with time at the end to discuss other ethical issues on people’s minds.

  • Participants will be able to differentiate different approaches to medical ethics
  • Participants will gain a deeper understanding of futility, consent, research ethics, and the ethics of artificial intelligence systems in radiology to apply to their practice

Lecture:  NanoEngineering gone viral: plant virus-based contrast agents and therapeutics

 

Abstract:

 

Nanoscale engineering is revolutionizing the way we detect, prevent and treat diseases. Viruses are playing a special role in these developments because they can function as prefabricated nanoparticles. We utilize and build-upon the high-precision assemblies of the viral capsids and utilize them as platform technologies, engineered and repurposed for a desired function. More specifically, we turned toward plant viruses as a platform nanotechnology. We have developed a library of plant virus-based nanoparticles and through structure-function studies we are beginning to understand how to tailor these materials appropriately for applications targeting human, veterinary and plant health. Through chemical biology, we have developed virus-based delivery systems carrying medically relevant cargo enabling tissue-specific imaging and treatment. We have developed contrast agents for MR imaging achieving r=80 mM­-1 s-1 and enabling imaging of cardiosvascular disease and cancer in mouse models. A particular exciting avenue is the development of plant virus-like particle platforms for cancer immunotherapy. The idea pursued is an ‘in situ vaccination’ to stimulate local and systemic anti-tumor immune responses to treat established disease, and most importantly to induce immune memory to protect patients from outgrowth of metastasis and recurrence of the disease. In light of the COVID-19 pandemic, we have also turned toward the development of diagnostic tools and vaccine candidates.

 

In this lecture, I will highlight engineering design principles employed to synthesize the next-generation contrast agents and nanotherapeutics using plant virus-based platform technologies, and I will discuss the evaluation of such in preclinical mouse models and canine patients.

Abstract: Chemical exchange saturation (CEST) MRI promises novel and non-invasive molecular image contrast with correlation from pH, over metabolites, to proteins and their conformation. A basic introduction of the phenomenon is followed by standard approaches and tricks on how to obtain the small CEST signals in a reproducible manner. Also with regard to ultra-high fields, where CEST has many benefits, but several challenges. Some axes provide hierarchical responsibility for faculty affairs, while others are focused thematically on clinical service delivery and other functions necessary for advancing non-clinical domains.  Leadership development and mentoring programs are sound investments in future leaders, and wellness programs insure a stable and healthy workforce. Dr. Ramachandran's major areas of research are: cognitive neuroscience, behavioral neurology - the study of cognitive and perceptual deficits in human neurological patients, neural plasticity and "phantom limbs", stroke rehabilitation, human visual perception/cognition, and visual psychophysics.

 

ABSTRACT: 

There is still scope for very simple low -tech experiments in  neurology.  Many of the experiments I’ll tell you about could have been done by Aristotle ( and certainly by Broca or Charcot ) but they weren't.

In this spirit we will try revive the Golden age of neurology harking back to Paul Broca, Freud (Yes , Freud !) Goldstein, with the torch being handed down by  Norm Geschwind to a Galaxy of modern  neurologists.

I will discuss five sets of experiments:
1) phantom limbs - beyond mirrors

2) Interpersonal acquired referral of touch and gargalesthesia

3) apotemnophilia; somatoparaphrenia, etc
4) synesthesia- why F is blue and G is yellow

5) constructing calendars in your brain
 

We conclude by suggesting that your calendar is enshrined in a network connecting the angular gyrus via inferior longitudinal fasciculus with place and grid cells in hippocampus and entorhinal cortex. Even such notoriously nebulous topics like social affinity and personality might be partially mediated by the same structures

 

Two axiomatic foundations of our discipline are:

1 ) connections are hard wired ; no plasticity in adults
2) autonomous hardwired modules - performing serial hierarchical computations en route to higher centers  - culminating in motor output

3) The human brain evolved through the gradual  accumulation of  adaptations

All these axioms are wrong.

The sensorium is a fluctuating mosaic of activity in a dynamic equilibrium with the environment - with connections being modified constantly in response to environmental challenges. So called modules interact not only with other modules ( mirror feedback ) including  ones  earlier in the hierarchy , but with skin and bones ( CRPS ) and indeed with other brains ( mirror neurons ) And they evolved via the  misappropriation of structures evolved for other  functions  and fortuitous interactions between them.  The gill arches of fish morphed into 3 loose bones in the jaws of reptiles ( allowing  consumption of huge prey and low  BMR ) two of which  became the tiny ear bones - thus serving  hearing instead if chewing. This principle of co-opting pre-existing structures for novel functions is equally - if not more - true for the human  brain.

Many didactic lectures induce a cognitive load in learners out of proportion to the content that they need to learn (or can learn) during that teaching session.  This is due in part to the content, and in part to the way it is displayed or presented.  By reducing the cognitive load on our audience, we can increase long term retention of information.  This lecture briefly summarizes some of the science behind cognitive load as it relates to presentations, and identifies simple steps to reduce it while maximize learning.
